20 Days for Documentation or Information
The individual must provide documentation supporting good cause no later than 20 days following the claim date. The District Office supervisor may provide a written extension to this 20-day period if the individual has difficulty obtaining the necessary evidence.
45 days for DFA Decision of Good Cause
The District Office supervisor must approve or disapprove decision of good cause no later than 45 calendar days from the claim date. If an extension was granted to the individual for providing information, extend the time frame for the decision by the same number of days.
To compute time frames, the good cause claim date is the date Form 754B is completed at the District Office, or is received in the mail and date-stamped by the District Office. The claim date is considered to be zero when counting for the time frame.
