901.01 Completing an Application for NH Child Care Scholarship (FAM)

To complete an application for NH Child Care Scholarship, the individual must comply with all of the following:

• complete an official application*;

• provide all required verification of eligibility criteria within timeframes; and

• participate in an eligibility interview at a District Office.

Exception: 12 month EMA recipients, whose eligibility for EMA is based on loss of FANF financial assistance due to new or increased earnings or hours of employment of the principal wage earner and who were receiving NH Child Care Scholarship at the time the FANF cash case closed, are not required to file a new application.

Applicants found eligible for NH Child Care Scholarship must arrange for child care with an enrolled provider and return Form 2530, Provider Verification Form, to the District Office within 30 days from the application date, or NH Child Care Scholarship is terminated.
