Reinstate 12-month extended medical assistance after termination if the individual proves good cause for failure to return a timely completed report form or failure to be employed. Reinstate eligibility retroactively to cover any months that extended medical assistance was terminated. Use the same 12-month coverage period that went into effect when the financial or categorically needy medical assistance case terminated.
