Require a signed and dated statement from the individual’s employer and/or training site supervisor verifying the hours worked and/or in training for each day that an individual:
• worked any 4 hours between 10 PM and 6 AM; or
• was engaged in employment and/or training for 12 or more hours and used a single child care provider for that day.
