111.01 AR Acting in Place of the Individual (FSM)

In order to act in place of the individual in the eligibility process, the AR must be all of the following:

• designated in writing. This designation must include the name and address of the AR, the signature or mark of the household member making the choice (if able), and the date. If designated by mark, the mark must be witnessed by someone other than the AR.

• knowledgeable about the individual’s circumstances. The individual is responsible for the accuracy of any information provided by the AR. The individual is liable for any overpayment resulting from inaccurate information.
