The AR assisting the individual must sign the Form 800HR, HEIGHTS-generated Application for Assistance, or Form 800, Application for Assistance, and any other forms the AR assisted in completing, including the Form 811H, Eligibility Interview Summary. If able, the individual must also sign or mark each form.
The AR assisting the individual must sign the Form 778, Authorized Representative Declaration.
