235.05 When Residents Leave the Group Living Arrangement (FSM)

When a resident leaves the group living arrangement, the facility must do the following:

1.  Notify the District Office of the change.

2.  Provide the individual with Form 470, Change Report Form, if possible.

3.  Advise the individual to report changes in circumstances to the District Office within 10 calendar days.

Group living arrangements acting as AR must also do all the following:

1.  Give the household its Food Stamp ID card, and

2.  Give the household its full monthly allotment if no stamps have been spent on its behalf, regardless of when in the month the household leaves the facility, or

Give the household one-half of its monthly allotment if stamps have already been spent on its behalf and the household leaves the facility on or before the 15th of the month, or

Give the household any remaining portion of its allotment if stamps have already been spent on its behalf and  the household leaves the facility after the 15th of the month, or

Give the departing members their pro rata share, if several residents have been certified together as one household but only some household members are leaving.

3.  If a resident leaves the facility unannounced, return the individuals Food Stamp ID card and any unused food stamps to the District Office.
