State Performance Plan and Annual Public Report
State Performance Plan-Individuals with Disabilities Education Act, Part C
Part C Annual Performance Report
is a report on the status of services provided by early intervention programs for infants and toddlers with disabilities and their families. This report satisfies the requirements of the Individuals with Disabilities Education Act (IDEA), Part C and the State Interagency Coordinating Council (ICC) annual status report to the Governor. The Annual Performance Report provides information about how well the State is progressing toward the targets established in the NH State Part C Performance Plan. This report covers the period from 7/1/2010 - 6/30/2011.
Annual State Application Under Part C as Amended in 2004, Federal Fiscal Year 2012
is a public notice on the application by the Department of Health and Human Services for federal funds to plan for and provide services to infants and toddlers with disabilities ages birth through two years as provided for under Part C of the Individuals with Disabilities Education Improvement Act (IDEIA, reauthorized 2004), is provided during the period: February 10, 2012 to April 9, 2012.
You may notice that the FFY2012 Part C Application is quite different than in past years. This change is due to the publication of Part C of IDEA Regulations published in 2011. As a result of these changes, states are required to provide a description of different aspects of the Part C program. Attached below, in addition to the application, is a document labeled Section II A-11 that describes how we collaborate with other early childhood programs. This document relates to Section 2 A-11 in the application. There are several sections that require rule changes or have public notice requirements that can not be submitted with the application. As a result, there will be a number of opportunities for you to participate in updating Part C policies and procedures so that they will be brought into compliance with the new Part C regulations.
