Pre-Admission Screening Residential Review
The Pre-Admission Screening Resident Review (PASRR) unit determines whether or not an individual who has an active diagnosis of mental illness or mental retardation meets the criteria for admission to a nursing facility and may require specialized services. PASRR is required for all persons seeking admission to Medicaid certified nursing facilities, regardless of whether their stay at the nursing facility will be paid for by Medicaid, Medicare or with other resources.
What is needed to make a referral for application?
The current PASRR referral form and the PASRR Notice to Individuals Seeking Admission to Nursing Homes must be completed to initiate the review process.
When the individual will be requiring long-term care, the following documentation must also be submitted:
- The date(s) and type(s) of any surgical or orthopedic procedures performed;
- The individual's rehabilitation goals and the estimated length of medically necessary care;
- The physician's name and address and plan of medical care for the individual;
- The plan of nursing care;
- The individual service plan, individual family support plan, or plan of care;
- Functional assessments;
- Intake, history and physical (H&P), and consultation reports when the individual is coming from a hospital setting; and
- Any other information that is considered by the referring agent to be necessary to make an accurate determination.

How Eligibility Is Determined
Once all the required documentation is received, the information is reviewed within 9 business days and the referring agent and accepting nursing facility will be notified of the determination.
