NH Community Passport Program
The New Hampshire Community Passport Program (NHCPP) is a nursing home transition initiative made possible through the Centers for Medicare and Medicaid Services, Money Follows The Person (MFP) Rebalancing Demonstration Grant. This program is designed and implemented to reflect a person-centered approach to service planning and delivery and providing greater opportunities for individuals to direct their own care to the extent that they choose. These are also the values inherent in the State's long term care vision statement.

Persons who are eligible for home and community based services and reside in qualified institutions will be eligible to participate in the Community Passport program. This includes person's eligible for the developmental disabilities waiver, the acquired brain disorder waiver, the elderly and chronically ill waiver, state funded mental health services and children eligible for the In Home Support Waiver.

This special initiative will focus on:

  • Individuals who have been residing in qualified institutions for a minimum of 3 months.
  • Individuals who have been receiving Medicaid for at least 1 day prior to discharge.
  • Individuals who voluntarily want to transition back into the community and are willing to play an active role in their plan of care.

The goals of the program are:

  • To increase the use of home and community-based long term care services.
  • To eliminate barriers that prevent or restrict the flexible use of Medicaid funds to enable Medicaid-eligible individuals to receive support for necessary long-term services in the settings of their choice.
  • To increase the ability of the State Medicaid Program to assure continued service of home and community-based long term services to eligible individuals who choose to transition from an institution to a community setting.
  • To ensure that procedures are in place to provide quality assurance for individuals receiving Medicaid home and community-based long term care services and to provide for on-going improvement in such services.

Transitional Support Services

In addition to services already offered under New Hampshire's HCBC waiver programs, the program will make supplemental and demonstration services available to ease the transition back into community living. These services include:

  • HCBC demonstration services
  • Health and safety assurances
  • Home Technology
  • Independent Living Skills
  • Vehicle Modifications
  • TeleHealth monitoring equipment
  • Products for the maintenance of health and hygiene
  • Certified Service Animals and related training for owners

Supplemental Demonstration Services

  • Supportive services/items for family to assist in transition
  • Home cleaning
  • Pest eradication
  • Medication bridge to avoid interruption of medication
  • Security deposits
  • Utility hookups
  • Necessary household purchases
  • Overnight visits to new home
