How Do I Pick A Health Plan?

  • Call the Enrollment Call Center toll free at 1-888-901-4999
  • Online through NH EASY!
  • Complete and return a Health Plan Enrollment Form

You do not have an NH EASY account yet? You can create one and pick your Health Plan online! The online process is easy and fast! All you have to do is:

  • Go to nheasy.nh.gov
  • Create an account. You will need this Personal Identification Number (PIN). This is printed on the letter you will get in the mail as part of your enrollment packet. You can always request your PIN too and it will be sent to your mailing address on file.

Once you have an NH EASY account:

  • Login by entering your User ID and password
  • Once you have logged in, select the "Go Now" button in the Care Management box. It's that simple!

If you do not create an account and pick your Health Plan online, call the Enrollment Call Center toll free at 1-888-901-4999 or complete and return your Health Plan Enrollment Form.

Have a Primary Care Provider PCP? When you pick a plan you will also be asked if you have a PCP. A PCP is the provider that will coordinate the majority of your care in the health plan. They will also be the primary provider you see for regular check-ups and when you are sick. In most cases a PCP is a:

  • Family/General Practitioner;
  • Internal Medicine;
  • Pediatrician;
  • OB/GYN;
  • Registered Nurse Practitioner; or
  • Physician Assistant (under supervision of physician).

You can choose your current provider as a PCP if they work with the health plan.

IT IS IMPORTANT TO PICK A HEALTH PLAN AND PCP.

If you don't pick a Health Plan by the due date in the letter, the Department of Health and Human Services (DHHS) will pick a Health Plan for you. We will notify you of the plan we pick.

If you don't choose a PCP, your health plan will pick one for you.

WHAT HAPPENS NEXT?

  • Your Health Plan will send you information about getting your health care from the Health Plan providers. The packet will explain the services your Health Plan provides.
  • Your Health Plan will call you. Your Health Plan also wants you to have your own primary care provider (PCP). During the call, they will confirm your PCP. They will also ask you questions about your health so they can better serve you. You can ask questions during this call.
  • Your Health Plan will send you a new card.

REMEMBER you still have NH Medicaid. You benefits are the same as before. Additional benefits will depend upon the Health Plan you picked. You can keep your current provider as long as he or she is in the Health Plan you picked.

If you get long-term care (LTC) supports and services, meaning you are in a nursing facility or get Home and Community-Based Care (HCBC) or Choices for Independence (CFI) services, your LTC services will continue to be covered through NH Medicaid in the same way they are now.

SHOW BOTH YOUR MEDICAID CARD AND HEALTH PLAN CARD WHEN YOU GET MEDICAL CARE! Still use your Medicaid card even when you get another card from your health plan. You will show both cards to your doctor each time you go for services.
