How Do I Know If My Waiver Or Nursing Facility Services Are Affected?

If you are receiving services under the Choices for Independence (CFI) Waiver, Developmental Disability (DD) Waiver, Acquired Brain Disorder (ABD) Waiver, or In-Home support (IHS) Waiver or Nursing Facility services, these services will not change.

You will still have the same providers for your waiver services. Your waiver services will continue to be covered through NH Medicaid in the same way they are now.

However, you will still need to pick a Health Plan for your medical services. That plan will help you get the medical care that you need to stay healthy. After you pick a plan, you will get all of your medical services, like doctor visits, from the network of Health Plan providers.

When you pick a plan you will get a new Health Plan card in addition to your regular Medicaid card.

Remember it is important to keep both cards. Present your regular Medicaid card and the new Health Plan card when you get a medical service.
