• Make a list of the doctors, clinics, hospitals, and other providers you use now. Decide which ones you like or are near where you live and which ones you want to keep using.
• Find out which health plans the providers work with:
  • View the Provider Directory
  • Call the Care Management Enrollment Call Center toll free at 1-888-901-4999
  • Call your doctor or provider
• If you want to keep seeing the providers you use, you must pick a Health Plan that works with your providers.
• If your providers work with more than one health plan, pick the Health Plan you like best.

If you do NOT have a doctor:
Compare the health plans and see which health plans offer the services you want. All health plans offer the same basic benefits and health management programs but some health plans might offer additional optional services and programs and extras for healthy choices.
